What Is It?
Neck pain (cervical pain) is extremely common and can significantly impact daily life, affecting your ability to drive, work at a computer, and even sleep. The cervical spine is remarkably flexible but also vulnerable to injury and degeneration. Common causes include cervical disc herniation, degenerative disc disease, cervical spondylosis (arthritis of the neck), muscle strain and tension (often from prolonged posture), whiplash injuries, and cervical radiculopathy (pinched nerve). Symptoms may include pain and stiffness in the neck, headaches originating from the base of the skull, and pain or numbness radiating into the shoulder, arm, or hand when nerve involvement is present.
How Is It Diagnosed?
Dr. Zhou evaluates neck pain with a comprehensive approach. The physical examination includes assessment of cervical range of motion, palpation for muscle tenderness and spasm, neurological testing of the upper extremities (reflexes, strength, sensation), and specific maneuvers like Spurling's test to check for nerve root compression. When nerve involvement is suspected, EMG and nerve conduction studies help determine which nerve root is affected and the severity of the compression. Cervical MRI provides detailed images of the discs, spinal cord, and nerve roots. X-rays may reveal alignment issues, arthritis, or instability. For persistent symptoms with nerve involvement, cervical epidural steroid injections or nerve root blocks can provide significant relief. BOTOX injections are effective for chronic neck pain with associated muscle spasm. Traction therapy may help decompress the cervical spine.
